Crosslinking with riboflavin and UV-A light has proven to be a first-line treatment for people with eye conditions such as Keratoconus, corneal weakness (ectasia).
This riboflavin/UVA therapy has been safe thus far. There have been no reported complications in the crystalline lens or retina due to the limitation of UVA transmission through the cornea. In a 3-year study of patients with actively pro- gressing keratoconus, the pattern of increasing mean keratometry values was not only halted, but it was also reversed and flattened by a mean of 2.00D.That result persisted during the 3-year follow-up period.
Ultraviolet light is used to promote increased cross-linking between collagen fibers within the cornea. Increasing the amount of cross-linking results in increased strength or rigidity of the cornea (figure below).
What to expect during the CXL?
The crosslinking procedure takes 60 to 90 minutes in most cases. Your doctor will place you in a reclining position and remove the epithelial layer from your cornea. Riboflavin eye drops will then be placed. After enough 30 minutes, the doctor will evaluate your eye to ensure that the cornea has sufficient riboflavin present.
The corneal thickness will be checked, and then the UV light will be applied for another 30 minutes.The you are asked to wear a bandage contact lens for five days. Adherence to postoperative instructions and use of anti-inflammatory eye drops are essential for the success of the procedure.
What is the rate of success?
Postoperative results become evident within one week and continue to improve over the next three to 12 months. Some patients may experience frequent changes in their corrective eyewear prescription until the cornea becomes fully stable. Most patients who undergo this treatment are able to prevent further vision damage from keratoconus. In some cases, vision may improve.
When can I wear contact lenses again?
You may return to wearing lenses after one month. Your lenses may need to be changed if your cornea changes shape.
Are there any risks?
As the mucous lining of the cornea is disrupted, there is a very small risk of infection. This is rare and prevented through the use of antibiotics before and after the procedure.

Is Ultraviolet light harmful?
Ultraviolet A light used in this procedure is not harmful to the eye in measured doses. Ultraviolet C light (in sunlight) is potentially harmful. The Light emitting diodes used in the 3C-R device is of a wavelength that is not harmful. Furthermore, light emission is carefully measured and calibrated prior to each treatment. There is also a self diagnostic check on the device which prevents use in case of a malfunction. There have been concerns about toxicity to the retina (at the back of the eye) however the Riboflavin pigment in the cornea and front of the eye, absorbs the Ultraviolet light and in effect stops the light from being transmitted to the retina.
